Founded in Chicago, Illinois, Vive Tu Vida En Forma was created in response to a growing need for culturally relevant health education, wellness resources, and equitable access to healthcare services within underserved communities.
For more than a decade, Vive Tu Vida En Forma has worked at the intersection of health equity, community engagement, and public health advocacy, helping individuals and families navigate complex healthcare systems while promoting healthier lifestyles through education, prevention, and early intervention.
What began as a grassroots effort to connect Latino families with health information and resources has evolved into a national community wellness platform serving diverse populations across the United States, including Latino, Black, Asian, Indigenous, Middle Eastern, immigrant, rural, and other historically underserved communities.
Vive Tu Vida En Forma is a community-based nonprofit organization dedicated to improving health outcomes and reducing health disparities through culturally responsive education, preventive health initiatives, community engagement, workforce development, and health equity advocacy.
